1. Scope of Telehealth Services
Man UnPaused Telehealth Clinic ("we," "us," or "our") provides telehealth-based healthcare services specializing in the diagnosis, treatment, and ongoing management of andropause (also known as male menopause or late-onset hypogonadism). Our services include:
- Virtual consultations with board-certified healthcare providers
- Comprehensive hormone panel laboratory testing
- Personalized treatment plans including hormone replacement therapy (HRT)
- Prescription management and medication adjustments
- Ongoing monitoring and follow-up appointments
- Nutritional guidance and lifestyle optimization recommendations
Our telehealth services are currently available to patients residing in the states where our providers are licensed to practice. By using our services, you acknowledge that telehealth has inherent limitations compared to in-person care and that certain conditions may require an in-person evaluation or emergency care.
2. Patient Responsibilities
As a patient of Man UnPaused, you agree to the following responsibilities:
- Accurate Information: You will provide truthful and complete information about your medical history, current medications, symptoms, and health conditions.
- Identity Verification: You confirm that you are the individual seeking treatment and will provide valid identification as requested.
- Age Requirement: You are at least 18 years of age.
- Follow-Up Compliance: You will attend scheduled follow-up appointments and complete recommended laboratory testing as directed by your provider.
- Medication Compliance: You will use prescribed medications only as directed and report any adverse effects promptly.
- Emergency Care: You understand that Man UnPaused does not provide emergency medical services. In case of a medical emergency, you will call 911 or visit the nearest emergency room.
- Technology Requirements: You are responsible for maintaining a reliable internet connection and a device capable of supporting video consultations.
3. Payment Terms
Man UnPaused operates on a transparent, straightforward pricing model:
- Initial Evaluation: $75 for your first comprehensive evaluation, including provider consultation, symptom assessment, and treatment plan development.
- Monthly Membership: $150 per month for ongoing care, which includes follow-up consultations, prescription management, lab review, and provider access.
- Laboratory Testing: Lab work is ordered through third-party laboratories (such as Quest Diagnostics or Labcorp) and may be billed separately. Many labs accept insurance for testing costs.
- Medications: Prescription medications are filled through licensed pharmacies and are billed separately from our service fees.
Payment is due at the time of service. We accept major credit cards and debit cards. All fees are clearly communicated prior to service delivery with no hidden charges.
4. Cancellation Policy
We believe in flexibility and patient autonomy:
- Membership Cancellation: You may cancel your monthly membership at any time with no cancellation fees or penalties. Cancellation takes effect at the end of your current billing period.
- Appointment Cancellation: We request at least 24 hours notice for appointment cancellations or rescheduling. Repeated no-shows may result in a rescheduling fee.
- Refund Policy: Fees for completed consultations are non-refundable. If you cancel your membership mid-cycle, you will retain access through the end of your paid period.
5. Limitation of Liability
To the fullest extent permitted by applicable law:
- Man UnPaused provides telehealth services on an "as-is" basis and makes no warranties, express or implied, regarding the outcomes of treatment.
- Individual results may vary. We do not guarantee specific health outcomes, symptom resolution, or treatment effectiveness.
- Man UnPaused shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from the use of our services.
- Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific service giving rise to the claim.
- We are not responsible for disruptions in service caused by internet connectivity issues, third-party platform outages, or circumstances beyond our reasonable control.
6. HIPAA Compliance
Man UnPaused is committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996 and its implementing regulations:
- All patient health information is stored and transmitted using HIPAA-compliant systems with encryption at rest and in transit.
- Our telehealth platform meets HIPAA security requirements for electronic protected health information (ePHI).
- Staff members receive regular HIPAA training and are bound by confidentiality agreements.
- We maintain a designated Privacy Officer responsible for HIPAA compliance oversight.
- You have the right to access, amend, and request restrictions on your health information as outlined in our Privacy Policy and HIPAA Notice of Privacy Practices.
7. Governing Law
These Terms of Service shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law provisions. Any disputes arising from or related to these terms or our services shall be subject to the exclusive jurisdiction of the state and federal courts located in Duval County, Florida.
If any provision of these Terms is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary so that the remaining provisions remain in full force and effect.
8. Changes to These Terms
We reserve the right to modify these Terms of Service at any time. Changes will be posted on this page with an updated "Last Updated" date. Continued use of our services after changes are posted constitutes acceptance of the revised terms. We encourage you to review these terms periodically.
